TOMAGWA HealthCare Ministries is now offering a Sliding Fee Discount Program effective October 12, 2021 for Medical services provided to the uninsured and underinsured.
Existing patients whose information is current will automatically be enrolled in the SFDP. All others must complete eligibility paperwork to apply.
The Sliding Fee Discount Program allows TOMAGWA to reduce or “slide” the fees for care at any of the TOMAGWA locations for anyone seeking services. You can apply to the program if you do not have insurance or if you do have insurance and need assistance to help pay for your care.
Our new sliding fee scale works by providing lower income patients and families with larger discounts based on Federal Poverty Level (FPL) guidelines. Levels A through E will receive a discount on services, with Level A patients receiving the largest discount. Level F designated patients (over 200% of FPL) will be required to pay 100% of charges.
